چکیده
Rheumatoid (RA) and Psoriatic arthritis (PsA) are characterized by extensive synovitis resulting in bone destruction in both diseases and new bone formation in PsA. Objective: This prospective study analyzed interleukin (IL)-6, IL-17, IL-23, tumor necrosis factor (TNF)-α serum and synovial fluid (SF) levels and osteoprotegerin (OPG), bone-specific alkaline phosphatase (BAP) and D-vitamin (D-Vit) serum levels in RA and PsA patients before and after anti-TNF-α. Methods: 15 RA and 15 PsA patients with knee effusion and starting adalimumab (ADA) treatment were enrolled. Serum and SF from inflamed knee joint samples were obtained at baseline and after 24 weeks of treatment. Statistical analysis was performed using Graph Pad 5.0 statistical software. Results: In RA and PsA, SF IL-6, IL-17, and TNF-α were higher than serum levels while higher serum IL-23 was detected than in SF. Higher serum/SF IL-6, IL-23 and SF TNF-α were observed in RA than PsA, while serum IL-17 was higher in PsA than RA. Positive feedback among IL-6, IL-23, IL-17 and CRP and negative correlation between IL-17 and TNF-α in RA were found. In PsA, IL-23 correlated positively with IL-6. OPG correlated positively with ESR/CRP in RA and PsA. D-Vit correlated negatively with OPG and ESR in PsA. Higher BAP in RA than PsA and positive correlation with IL-6 in RA and with OPG in PsA were noticed. In PsA, IL-6 and OPG serum levels decreased significantly after 24 weeks ADA treatment. Conclusions: Distinct distribution of inflammatory cytokines and correlations with bone mediators were detected in RA and PsA. ADA affected inflammatory cytokines and bone mediators in PsA patients. Comparing bone mineral density in rheumatoid arthritis and psoriatic arthritis
Background: Changes in patients’ bone mineral density (BMD) is one of the problems in patients with rheumatoid arthritis, which can be due to the use of corticosteroid drugs to reduce patients' symptoms or due to the aging process in patients. In this study, we decided to compare BMD in patients with psoriatic arthritis and rheumatoid arthritis. Methods: This study as a cross-sectional descrip...
متن کاملMajor cost savings associated with biologic dose reduction in patients with inflammatory arthritis.
The purpose of this study was to explore whether patients with Inflammatory Arthritis (IA) (Rheumatoid Arthritis (RA), Psoriatic Arthritis (PsA) or Ankylosing Spondylitis (AS)) would remain in remission following a reduction in biologic dosing frequency and to calculate the cost savings associated with dose reduction. This prospective non-blinded non-randomised study commenced in 2010. Patients...
متن کاملEvaluation of COVID-19 incidence in psoriatic patients receiving Adalimumab
Background: Adalimumab is a highly human monoclonal antibody that binds to tumor necrosis factor, a key proinflammatory cytokine pathogenic in psoriasis. It considers an influential factor in controlling moderate to severe psoriasis. This study aimed to evaluate the impact of Adalimumab on COVID-19 incidence in patients with psoriasis. Methods: This cross-sectional study was performed at Baqiy...
متن کاملBone mineral density changes during treatment of rheumatoid arthritis with disease-modifying-anti-rheumatic drugs
Background: Bone mineral density (BMD) changes during the course of rheumatoid arthritis (RA). The present study was designed to investigate the status of BMD in patients with RA treated with anti-rheumatic drugs. Methods: BMD at the femoral neck (FN-BMD) and lumbar spine (LS-BMD) were measured by dual energy x-ray absorptiometry (DXA) method using Norland densitometer. Disease activity (DA) wa...
متن کاملDermatofibrosarcoma protuberans (DFSP) with fibrosarcomatous changes in a patient with psoriasis treated with adalimumab
P soriasis is a Th1/Th17 chronic immunemediated disease, characterized by increased levels of tumor necrosis factor (TNF)-alfa. TNF-alfa blockers including adalimumab have been widely used to treat immune-mediated diseases such as inflammatory bowel disease, psoriasis, rheumatoid arthritis, and psoriatic arthritis. TNF-alfa plays a central role in the inflammation and cellular immune response a...
